We have a dedicated emergency helpline to make sure that you can get the necessary assistance when it is most needed. Call the 24 hour Liberty Helpline at (65) 6334-4434 when you need EMERGENCY:

  • Home Assistance for accidents at home involving personal health risk/physical damage to property and disruption to essential services such as water, electricity, gas and mains.
  • Medical Assistance while traveling overseas anywhere in the world.

MOTOR CLAIMS

MEDICAL - HOSPITAL & SURGICAL CLAIMS ( proMedico, proMedicare, CityCare)

Submit the following original documents in support of your claim to us when making a claim for injury, sickness or surgery:

  1. Duly completed Liberty Medical Claim Form: with Section A completed by you and Section B completed by the attending doctor
  2. Inpatient discharge summary (ie. if you are admitted to a restructed hospital)
  3. Final and itemized hospital bill from the hospital for the admission
  4. All other original hospital bills where appropriate such as Pre-hospitalisation/diagnostic and Post-hospitalisation/follow-up bills.

TOURCARE PLUS CLAIMS

Submit the following documents in support of your claim:

  1. Duly completed Liberty Tourcare Plus Claim Form
  2. Copy of airline ticket or boarding pass or the relevant page(s) of the passport showing entry into or exit from the country of visit
  3. Specific documents relevant to the nature of your claim - please refer to Liberty Tourcare Plus claim form for these documents

OTHER NON-MOTOR CLAIMS

For non-emergency situations, you can help us to ensure that we can attend to your needs expeditiously by:

  1. Notify us immediately of the incident giving rise to the claim.
    You can either inform us through your insurance agent/broker or call us directly at Tel: (65) 6221-8611 Fax: (65) 6224-1047 or e-mail us at claims@libertyinsurance.com.sg

  2. Submit all your original documents in support of your claim to us.
    Documents such as police report, medical report and bill, repair bills, photographs etc. will help us to speed up our claim process.

  3. If third party is involved…..
    • Give us details of the third party
      (including phone / contact number)
    • Preserve your legal position by NOT ADMITTING to any liabilities. We have a pool of independent adjusters and lawyers to handle third party claims and to ensure that we achieve a realistic and fair settlement for you.
