MOTOR CLAIMS
What should I do when my
vehicle is involved in an accident?
MEDICAL - HOSPITAL &
SURGICAL CLAIMS ( proMedico, proMedicare, CityCare)
Submit the following
original documents in support of your claim to us when making a claim
for injury, sickness or surgery:
- Duly completed Liberty
Medical Claim Form: with Section A completed by you and Section B
completed by the attending doctor
- Inpatient discharge
summary (ie. if you are admitted to a restructed hospital)
- Final and itemized
hospital bill from the hospital for the admission
- All other original
hospital bills where appropriate such as Pre-hospitalisation/diagnostic
and Post-hospitalisation/follow-up bills.

TOURCARE PLUS CLAIMS
Submit the following
documents in support of your claim:
- Duly completed Liberty
Tourcare Plus Claim Form
- Copy of airline ticket or
boarding pass or the relevant page(s) of the passport showing entry
into or exit from the country of visit
- Specific documents
relevant to the nature of your claim - please refer to Liberty Tourcare

OTHER NON-MOTOR CLAIMS
For non-emergency
situations, you can help us to ensure that we can attend to your needs
expeditiously by:
- Notify us
immediately of the incident giving rise to the claim.
You can either inform

- Submit all your original documents in
support of your claim to us.
Documents such as
police report, medical report and bill, repair bills, photographs etc.
will help us to speed up our claim process.

- Preserve your
legal position by NOT ADMITTING to any liabilities. We have a pool of
independent adjusters and lawyers to handle third party claims and to
ensure that we achieve a realistic and fair settlement for you.
