CLAIMS (proMedico)
To file your claims under proMedico, submit the following original documents in support of your claim to us when making a claim for injury, sickness or surgery:

· Duly completed Liberty Medical Claim Form: with Section A completed by you and Section B completed by the attending doctor |

· Inpatient discharge summary (i.e. if you are admitted to a restructured hospital) |
|
· Final and itemized hospital bill from the hospital for the hospitalization |
|
· All other original hospital bills where appropriate such as pre-hospitalization / diagnostic and post-hospitalization / follow-up bills |
