If you are claiming for Personal Accident, duly complete the following:

·      Duly completed Liberty Personal Accident

·      Original medical receipt/tax invoice (require treating doctor to state medical diagnosis on the receipt/tax invoice)

·      Or page 2 of Personal Accident Claim Form (medical information – completed by the
attending doctor)

If you are claiming for Medical, duly complete the following:

·      Duly completed Liberty Medical Claim Form: with Section A completed by you and Section B completed by the attending doctor

·      Inpatient discharge summary (i.e. if you are admitted to a restructured hospital)

·      Final and itemized hospital bill from the hospital for the hospitalization

·      All other original hospital bills where appropriate such as Pre-hospitalization/diagnostic and Post-hospitalization/follow-up bills
