To file for your PACare Plus claims, submit the following documents in support of your claim:

· Duly completed Liberty Personal Accident Claim Form |

· Original medical receipt/tax invoice (require treating doctor to state medical diagnosis on the receipt/tax invoice) |

· Or page 2 of Personal Accident Claim Form (medical information – completed by the attending doctor) |
